I know it happened to MS in 94 for passing on the installation lap. Maybe it was England?? Anyway I don't recall anyone disobeying a black flag since. If anyone has, I would bet it was MS again anyway LOL.

He did, but the correct answer was Andrea Montermini at Monaco 1995 when he did a jump start and didn't come into the pits to take his stop go.

B. Don't know which race it was, but was it with Alfa Romeo? I know it was in 1950, but I don't ever remember reading about which grand prix it was. I do know that it was the second round of the championship, if that counts?
