What's wrong with that? They have the knowledge and the education on the best interests of a human body, but as an individual they can choose how to live their own life right? It depends on their clients/patients on taking their advise or not. If someone doesn't want a fat doctors advise, just don't go to him...it's a free world.
It's your business if you're breathing in their 2nd hand smoke or if you're stuck on a hospital waiting list behind someone who chose to take up smoking in an age when all the health implications are known to everyone.
