Not one person is to blame. Hamilton should have done as he was told and Alonso shouldn't have done what he did. The FIA used the whole situation as an excuse to penalise McLaren for no reason.

LH's fault because ignored the team orders, FA's fault because stopped deliberately LH, RD's fault because tried to cover it up, Fia's fault for punishing McLaren with no reason.
